Current and Former Students

Please work with your academic advisor to make any changes to your schedule. See the Academic Calendar for registration deadlines. If you do not know who your academic advisor is, please email Registrar@CUChicago.edu and include your H number so we can direct you to the appropriate advising office.

Please log into CUConnect > Resources > search the Repository for and submit the “Student Right to Privacy Release Form.”

Please log into CUConnect > Resources > search the Repository for and submit the “Incomplete Grade Authorization” along with any supporting documentation. The student must meet the conditions to be eligible, the instructor must consent, and the form and documentation to support the request must be received by the Office of the Registrar by the deadline.

Please log into CUConnect > Resources > search the Repository for and submit the “Grade Appeal Form."

Unofficial transcripts are available to current students via CUConnect under the "Academics" tab. If you cannot access your unofficial transcript, please fill out the Request for Unofficial Transcript form.

Concordia University Chicago has authorized the National Student Clearinghouse to process transcript orders on its behalf. Request an official transcript and refer to the steps found in our user guide.

Concordia University Chicago has authorized the National Student Clearinghouse to provide degree verifications on its behalf. Students may request degree verification in one of two ways:

If you do not know who your academic advisor is, you can find your advisor(s) name(s) in DegreeWorks, or please email Registrar@CUChicago.edu and include your H number so we can direct you to the appropriate advising office.

If you have recently graduated, your diploma will automatically be ordered at the end of the final audit time frame. Please refer to the prospective completer communication sent by email upon the submission of your intent to graduate/complete for exact dates.

If you would like an ADDITIONAL copy of your diploma, please complete the Diploma Reorder Request Form and send it to Registrar@CUChicago.edu.

Only degree-seeking students (bachelor’s, master’s, education specialist, doctoral) will be invited to participate in the commencement ceremony and receive a diploma. Please note diplomas will only list the degree conferred (e.g. Bachelor of Arts, Master of Science, Doctor of Education, etc.) and will not include the major/program (e.g. history, applied exercise science, etc.). Both your degree and major/program information will be reflected on your transcript.

Please submit the In School Loan Deferment form to Registrar@CUChicago.edu for processing.

Please complete the Change of Personal Information Form and send it to Registrar@CUChicago.edu.

Please log into CUConnect > Resources > search the Forms Repository for and submit the “Change of Grade Incomplete to Grade” fillable form.

If you are an academic or faculty advisor, complete the Degree Works Update Request Form. You may also find the web form by logging into CUConnect, going to the Resources tab, and in the Repository searching for and submitting the “Degree Works Update Request.”
